In order to sign for one or more classes, please fill the form below.
If you would like to sign in several persons, e.g. for your children, please use one form per person.
Please indicate a telephone number (mobile, office, home) where you could be contacted during the day. We only need it for the very exceptional case, when a class has to be cancelled, due to very bad weather, or health concerns of the teacher. Thank you very for also adding an e-mail address.

Membership fees for all the classes are to be transferred to the account of D.T.V Kielen IBAN LU92 0090 0001 6120 9630 at Caisse Centrale Raiffeisen CCRALULL (Code BIC)

IMPORTANT Please indicate on the transfer form the NAME(S) of the PARTICIPANT(S) and the NAME of the CLASS(ES), especially if the name used for signing in ist not the name of the bank account. |
Please note: To send the form, you will have to calculate the addtion or substraction under "Verification" and then "Fill in the result".
